Outcomes and impacts of the pilot phase

This publication brings together the main results of the pilot phase of the Gulbenkian Home Care initiative, presenting evidence on the impact of implementing an integrated home care model tailored to individual needs.

Based on the external evaluation of the 15 pilot projects supported by the Calouste Gulbenkian Foundation, it presents indicators relating to older people receiving care, informal caregivers, professionals and organisations, enabling an assessment of the intervention’s effects on quality of life, autonomy, well-being, workforce capacity and service transformation.

This publication aims to serve as a resource to support the improvement of practices and inform the development of public policies in the field of long-term care.
