Gulbenkian Home Care
The Gulbenkian Home Care initiative is based on the recommendations of the UN Decade of Healthy Ageing (2021-2030) and the unequal and asymmetrical access to senior care in Portugal.
The initiative is designed to encourage the implementation of outreach projects that preserve the intrinsic and functional abilities of older people through the provision of integrated and continued care in place, at home and in the community, with individual plans, provided by competent professionals, respecting the rights of older people.
Gulbenkian Home Care is being tested in 15 different regions on mainland Portugal and the islands, through pilot projects coordinated by different organisations that promote service and health and social care integration. These organisations receive financial support and technical monitoring from the Foundation, and support and training from MAZE Impact, with evaluation carried out by CINTESIS – Centre for Health Technology and Services Research of the Abel Salazar Biomedical Sciences Institute of the University of Porto.
Gulbenkian Home Care: Pilot Phase
The 15 projects supported in the pilot phase (2023–2024) developed activities for 950 older people and involved around 80 professionals.
Tempo de Cuidar 3.0 (Time to Care 3.0)
This project aims to broaden the scope of care that the organisation already provides in terms of physical and emotional health, certification and differentiation. Psychological support, physical therapy and occupational therapy is now also offered alongside medical and nursing care.

This project offers an integrated response that prioritises digital transformation, team training and supervision, and the introduction of technological solutions for monitoring health parameters, with a view to improving care, autonomy and quality of life.

Cada Casa é um caso (Each person is unique)
This project aims to provide neighbourhood services, with longer opening hours, focusing on specific areas such as health, rehabilitation, cognitive stimulation (at home or in social spaces in the community), social support and training for formal and informal carers.

S@RKIS: A smart platform for seniors and their carers
This project aims at developing an integrated home care model with different services and areas of expertise, including the creation of a digital platform to strengthen the emotional dimension, training, information and the empowerment of users and their carers.

Em Casa Con’Vida (Dignified living at home) – Support for older people and their carers in a home setting
This project focuses on forming multidisciplinary teams, suited to the identified physical, mental and social needs of older people and their informal carers. The aim is to combat the social isolation of older people and their carers through personalised actions in the home.

Home 360
This project aims at developing a specialised community/home response for people with dementia and their carers. The goal is to avoid early institutionalisation, to promote proximity, accessibility and continuity of care, to encourage links with community resources, and to empower people with dementia and their carers in adapting to the disease.

Dar Continuidade (Ensuring Continuity)
This project aims at increasing the number of users covered and diversifying care by providing telecare services, equipment and materials to support functionality and autonomy (technical aids), psychological support, and training for carers.

Kinisuru| EnvelheSer (Dignified Ageing)
This project aims to ensure and extend the support already provided, guaranteeing an individualised care plan, provided by an interdisciplinary team that integrates health and social care (social worker, psychologist, speech therapist, occupational therapist, nurse).

This project aims at diversifying the range of care provided to improve users' functional abilities and reduce hospitalisation and emergency room visits. It hopes to achieve these goals by using technological solutions for monitoring users and training carers.

Sorrisos ao Domicílio (Smile at Home)
This project aims at diversifying the range of care provided by the home care service, through physical and cognitive stimulation and rehabilitation sessions, socialising and sharing among peers, and walks. It also includes theory-based and practical training, capacity-building and information sessions.

This project aims to improve the physical and mental health of older people, preventing illness, by forming a multidisciplinary team and providing physical therapy and psychological support, including individual and group monitoring and rehabilitation.

My SAD – O meu Serviço de Apoio Domiciliário (My Home Care Service)
This project aims to provide an innovative response for home care, focusing on geriatric psychomotricity. It also aims to develop the "Cuidadora São” system, a virtual carer, available via telephone, who tracks and monitors health data, therapies and emotional support.

Mértola_Cuida em casa (Mértola Home Care)
This project focuses on preventive care and the physical and mental health rehabilitation of home care service users by introducing occupational therapy, creating a network of volunteers who regularly visit users, and developing cultural activities in the community.

Gulbenkian Home Care 2.0
In 2025, a new call for proposals was launched to broaden support for innovative home care initiatives. By 2027, 14 new projects will be up and running across Portugal, from north to south.
Rostos Tejo
An integrated home care model for families of people with dementia in rural Ribatejo. It combines health and social care, specialist dementia support, professional training, and the creation of a Family Care Plan. The aim is to fill gaps in local provision and engage municipalities in developing integrated community-based solutions.

This project addresses the needs of dependent people and their informal caregivers, enhancing the skills of home care teams and coordinating with health services. It offers therapies, personal assistance, and 24-hour caregiver respite, adapting to each case to keep people at home with dignity and safety.

Developed in a predominantly rural setting, this project focuses on keeping older people at home safely and with dignity. By coordinating health and social services and using proximity technology, it aims to improve health, reduce caregiver burden, promote the digitalization of care, and reduce preventable hospitalizations.

Ser C.A.S.A.
Support for older people in the Algarve mountains, living in scattered and hard-to-reach villages with very basic living conditions. The project offers personalized services – tailored to each user – in physiotherapy, occupational therapy, nursing, and social support. It also includes health literacy activities and social gatherings, addressing needs in areas without similar services.

Com'VIVE SaudavelMENTE
A project providing integrated home health care, including physiotherapy, occupational therapy, and medical assistance. Building on a pilot project from the first phase of Gulbenkian Home Care, it will introduce assistive technology for monitoring older adults, a technical aid bank, and a community podcast. It will also continue to strengthen the skills of informal caregivers.

Leve(Mente) em casa
Specialized home care for early screening and diagnosis of dementia, with a multidisciplinary team providing assessments, cognitive and sensory stimulation, and medical referrals. It improves access to care in isolated areas, works with local health units, and combats caregiver isolation.

Cuidar com Sentido – Esperança no domicílio
Personalized home care centred on the individual, integrating health and well-being, psychosocial and relational support, and promotion of autonomy and participation. Care plans are co-designed with each beneficiary, and 24-hour support technology is provided, ensuring dignity at home.

Based on mapping needs and available services, this project proposes expanding and upgrading home care in Torres Vedras, focusing on empowering older people and caregivers and creating a “Social Facilitators Pool” made up of trained older adults.

Personalized service for older people, with a multidisciplinary team integrating immediate-response technology and supervision for complex cases such as dementia, diabetes, or cancer. It also includes psychological support, cognitive stimulation, socio-cultural activities, and caregiver respite.

SOS Home
Integrates technology to support those wishing to age at home, using intuitive systems adapted to low digital literacy for early intervention in cases of falls, disorientation, missed medication, dehydration, acute illness, or loneliness. It includes individualized care plans, close coordination with health services, and community involvement through volunteering.

Mértola_Cuida em casa 2.0
Provides home health and well-being care, uses ALICE4U technology (Ageing Longer Intelligent Care Environment), and offers community support via telephone volunteering to combat isolation, along with ongoing staff training. The goal is an innovative, close, and humanised approach that values each person in their life context.

Mora(r) com Memória
Home Care Service specialized in supporting people with dementia, informal caregivers, and professionals, with a multidisciplinary team. Individual Care Plans include neurostimulation/functional rehabilitation and 24-hour support. In addition to improving local dementia services, it has potential for replication throughout Alentejo Central.

Mente sã, corpo feliz
Aims to promote autonomy and functional abilities in older people by providing home psychology, physiotherapy, or speech therapy. It strengthens team training, builds local partnerships with clinics, pharmacies, gyms, and health centres, and reinforces the Teleassistance Service of the Red Cross and the Technical Aid Bank.
