This project aims at providing assistance to older people who do not have a support system after being discharged from hospital.
The promoting association has vast experience in helping older people living in isolation in Lisbon city centre and its Mouraria neighbourhood, and works with a local collaboration network. The project includes coordinating with healthcare services while seniors are still hospitalised, mapping local services and offering personalised assistance for consultations, medical examinations and daily activities.
